Documents Required for Passport Name Change

• Current Passport: The original booklet you currently possess 

• Identity proofs such as Voter ID, PAN card, or Aadhaar card 

• A Joint Photo Declaration or a Marriage Certificate is required for marriage (Annexure J) 

• A certified copy of the divorce decree is required 

• Original Gazette Notification and cuttings of two newspaper ads (one in English and one in a local language) are required for a legal name change 

• Affidavits: A deed poll or self-declaration for a name change.
