   1. First two and last two pages of the passport including the ECR/Non-ECR page.
   2. Page of observation (if any).
   3. Validity extension page in respect of Short Validity Passport (if any).
3. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
4. Documentary proof for any one of the Non-ECR categories.

Renewal of Short Validity Passport (SVP)

1. Old passport, original.
2. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page.
   2. Page of observation (if any).
   3. Validity extension page in respect of short validity passport (if any).
3. Proof of documents which eliminate the cause of issuance of Short Validity Passport (SVP).
4. Documentary proof for any one of the Non-ECR categories.
5. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.

Lost or Stolen Passport

1. Self-attested copies of the first two and last two pages of the passport including the ECE/Non-ECR page.
2. Proof of date of birth.
3. Proof of present address.
4. Original police report.
5. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI)letter as per Annexure H.
6. Documentary proof for any one of the Non-ECR categories.
7. Affidavit (as per Annexure F) stating how and where the passport was lost or damaged.

Damaged passport (Note: The passport number being readable, name legible and photograph intact)

1. Old passport, original.
2. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page.
   2. Page of observation (if any)
   3. Validity extension page (if any) in respect of Short Validity Passport (SVP).
3. Proof of date of birth.
4. Affidavit (as per Annexure F) stating how and where the passport was lost or damaged.
5. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
6. Documentary proof for any one of the Non-ECR categories.

Damaged passport (Note: If the passport is damaged beyond recognition)

1. Self-attested copies of the first and last two pages of the passport including the ECR/Non-ECR page.
2. Proof of date of birth.
3. Proof of present address.
4. Original police report.
5. Affidavit (as per Annexure F) stating how and where the passport was lost or damaged.
6. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
7. Documentary proof for any one of the Non-ECR categories.

Change in Appearance of the passport holder

1. Old passport, original.
2. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page.
   2. Page of observation (if any).
   3. Validity extension page (if any) in respect of Short Validity Passport (SVP).
3. Recent photograph of the passport holder showing his/her latest appearance. (Note: A notarised statement would be required to be submitted in case of request from Sikhs who want to change from turban photo to clean shaven ones or the other way round.)
4. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
5. Documentary proof for any one of the Non-ECR categories.

1. Change in signature of the passport holder
2. Change in name following divorce
3. Minor changes in the existing name of the passport holder
4. Addition of spouse's name following marriage
5. Deletion of spouse's name following divorce
6. Deletion of spouse's name following separation
7. Deletion of Emigration Check Required (ECR)

Change in name (in case of Government/PSU/Statutory body employees)

1. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page.
   2. Page of observation (if any).
   3. Validity extension page (if any) in respect of Short Validity Passport (SVP).
2. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
3. Documentary proof for any one of the Non-ECR categories.
4. Gazetted notification changing name in applicant's department.
5. Fresh Identity certificate in changed name.

Major changes in the existing name of the passport holder

1. Old passport, original.
2. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page.
   2. Page of observation (if any).
   3. Validity extension page (if any) in respect of Short Validity Passport (SVP).
3. Two public or school documents issued in the changed name to proof that the passport holder has actually changed his/her name.
4. Clippings of two local newspapers or the Gazette notification of the concerned state government.
5. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
6. Documentary proof for any one of the Non-ECR categories.

Change of date of birth

1. Old passport, original.
2. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page.

Change of spouse's name due to re-marriage

Change of Address

Change in place of birth involving change of state or country

3. Proof of place of birth.
4. Affidavit stating the reason for change in place of birth.

6. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
7. Order from the first class judicial magistrate or the Sub-divisional magistrate if change in date of birth is more than two years.
8. Certificate of citizenship by Ministry of Home Affairs.

Change in place of birth not involving change of state or country

1. Old passport, original.
2. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page
   2. Page of observation (if any)
   3. Validity extension page (if any) in respect of Short Validity Passport (SVP).
3. Proof of place of birth.
4. Affidavit stating the reason for change in place of birth.

Change in Sex

1. Old passport, original.

   2. Page of observation (if any).
   3. Validity extension page (if any) in respect of Short Validity Passport (SVP).
3. Sworn affidavit regarding change of sex.
4. Certificate from the hospital where the individual underwent sex change operation successfully.
5. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
6. Documentary proof for any one of the Non-ECR categories.

Change in Parent's name

1. Old passport, original.
2. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page.
   2. Page of observation (if any).
   3. Validity extension page (if any) in respect of Short Validity Passport (SVP).
3. No Objection Certificate (NOC) as per Annexure G or Prior Intimation (PI) letter as per Annexure H.
4. Documentary proof for any one of the Non-ECR categories.
5. Passport of parents with new name in original
6. Self-attested copies of the:
   1. First two and last two pages of the passport including the ECR/Non-ECR page.
   2. Page of observation (if any).
   3. Validity extension page (if any) in respect of Short Validity Passport (SVP).
7. Service record or pension order or property documents showing that parents had changed their name.
8. If parents are dead any document which proves that names were changed during their lifetime.
