    Form 1 Self Declaration is an important document that needs to be submitted while applying for Driving Licence. It basically checks whether the applicant is physically fit to drive and serves as a physical fitness declaration form.

    How to fill the RTO Form 1?

    The below mentioned points need to be filled on Form 1:

    • Point 1 & 2: Input your full name and your father’s name (as mentioned on the identity proof). Married women can use their husband’s name instead of father’s name. However, the same name should be present on the identity proof.
    • Point 3: Input your permanent address as mentioned on the address proof.
    • Point 4: In case you don’t have a permanent address, you need to fill your temporary address in this section.
    • Point 5: Input your date of birth as mentioned on your identity proof.
    • Point 6: In this section, you need to mention two identification marks. It can either birth mark, stitch or any visible scratch on the exposed portion of the body like leg, face or hand.
    • Point 'a': This point asks if you have history of epilepsy. Epilepsy is a medical condition that affects the central nervous system. In this case patients often experience unusual sensations and seizures. Mark this field ‘No’ if you have not experience anything like this.
    • Point 'b': This point asks if you have good eyesight. Mark this field ‘Yes’ if you can distinguish between number-plates of other vehicles in broad daylight from a distance of 25 metres.
    • Point 'c': This point asks if you have any muscular issues or motor disability. Mark this field ‘No’ if you have not experience anything like this.
    • Point 'd' & 'e': These points ask if you can differentiate between colours and not have colour blindness. Mark ‘no’ if you do not have any history of colour blindness and can differentiate between colours.
    • Point 'f': This point asks if you have good auditory skills. Mark this field ‘Yes’ if you can drive without using any hearing aid.
    • Point 'g': This is a generalised point and asks if you have any other disability or disease which can affect your driving skills and pose threat to co-passengers as well as people on road. Mark this field ‘Yes’ if you are physically fit to drive.

    1. Valid identity proof: Aadhar card, PAN card, etc.
    2. Valid address proof: Bank passbook, electricity bill, etc.
    3. Birth certificate.

    How to download Self Declaration Form 1?

    The Self Declaration Form 1 can be easily downloaded from the ‘Parivahan Saarthi’ website. Follow the procedure mentioned below to download the form:

    • Step 1: Visit https://parivahan.gov.in/parivahan/
    • Step 2: Choose the state you want to apply from.
    • Step 3: Click on ‘Others’ tab present at the top-right of the page.
    • Step 4: From the drop-down menu, select the ‘Print Application Form’ option.
    • Step 5: Fill the ‘Application number’ and ‘Date of Birth’.
    • Step 6: Download ‘Form1’.
