    What does EPF ‘Nomination’ mean?

    An employee has to not only provide his or her details but also details of nominees as per Employees Pension Scheme 1995, Employees' Provident Fund scheme 1952 and Employees' Deposit Linked Insurance Scheme. All information pertaining to the said schemes has to be furnished in form no. 2. For instance, an individual can nominate one or more persons of his or her family to get the provident fund proceeds upon death. Under Employee Provident Fund Scheme 1952, ‘family’ is includes husband/wife, offspring (married/unmarried, adopted), in addition to dependant parents. Nominees under Employees Provident Fund Scheme, 1952 receive the benefits of Employees Deposit Linked Insurance Scheme. Under Employees Pension Scheme,1995, family includes husband/wife and children. If an employee wishes to change his or her nominees under Employees Pension Scheme,1995, he or she should send a revised form no.2 via the employer.

    Types of EPF nomination forms

    There are three types of nomination forms as listed below:

    • Form No.2: This is a common form (pension and provident fund schemes) used for payment of fund amount to the nominees.
    • Form No.11: This form is a member’s declaration of being employed in a factory/establishment where both the Employees' Family Pension Fund Scheme and Provident Fund are in force.
    • Form No.9: This form is a declaration by an individual taking up work in a factory/ establishment where the Employees' Pension Scheme is applicable.

    What does Form No. 2 entail?

    Form No.2 is a nomination and declaration form under the Employees Pension and Provident Fund schemes (paragraph 18 of Employees Pension Scheme 1995 and paragraph 33 and 61 (1) of Employees Provident Fund Scheme 1952)

    EPF form no. 2 requires a member to fill in the following details

    • Name of the member
    • Father/husband’s name
    • Date of birth
    • Permanent address
    • Name of the nominee
    • Complete address of the nominee
    • Member’s relationship with the nominee
    • Date of birth of the nominee
    • Share of the provident fund amount to be paid to each nominee
    • In case the nominee is a minor, the member has to furnish details of the guardian who will receive the provident fund amount
    • In case the member does not have any dependent parents or family as mentioned under para 2 (g) of Employees Provident Fund Scheme 1952, he or she should strike out the sections which are not applicable
    • Signature of the subscriber
    • Details of family members who will receive the children/widow pension when the member dies. The member has to mention the name, age and address of the family members. The member should also mention the relationship with the nominees.
    • Name and address of the establishment/factory
    • Signature of employer/ authorised officer

    What does Form No. 11 entail?

    Form No.11 is a nomination and declaration form under para 24 of Employees Pension Scheme, 1995 and para 34 and 57 of the Employees Provident Fund Scheme 1952. Form No.11 requires a member to furnish some details as listed below:

    • Name of the member
    • Date of birth of the member
    • Father/Husband’s name
    • Mobile Number
    • Email ID
    • Individual hsa to mention is he or she was a member of Employees Provident Fund Scheme, 1952 and Employees’ Pension scheme 1995. If yes, then the individual has to fill out details regarding his or her previous employment such as listed below:
      • UAN or previous provident fund account ID
      • Date of exit
      • Previous scheme certificate number
      • Previous pension payment order
    • International workers have to specify the following:
      • Country of origin
      • Passport number
      • Passport validity
      • Educational qualification
      • Marital status
      • Any disabilities (visual, locomotive, hearing)
    • KYC details to be included are as follows:
      • Bank account number with IFSC code (mandatory)
      • Aadhar
      • PAN
      • Passport
      • Driving License
      • Voter ID
      • ESIC card
      • Ration Card

    All the aforementioned documents must be self-attested

    • Declaration by the present employer
    • Signature of the employer along with establishment’s seal

    What does Form No.9 entail?

    Form No.9 is a declaration form of Employees Pension Scheme,1995 (para 34) and Employees Provident Fund Scheme 1952 (para 34 and 57). This form is a declaration of a member taking up employment in an establishment where both Employees’ Provident Fund Scheme, 1952 and Employees’ Pension Scheme, 1995 are applicable. Form No.9 requires a member to furnish the following details:

    • Name of the member
    • Date of birth of the member
    • Father/Husband’s name
    • Mobile Number
    • Email ID
    • Individual hsa to mention is he or she was a member of Employees Provident Fund Scheme, 1952 and Employees’ Pension scheme 1995. If yes, then the individual has to fill out details regarding his or her previous employment such as listed below:
