What is Voter ID Form 8?

In India, Voter ID Form 8 is an official application used by registered voters to correct or update their personal information in the electoral roll. This form is essential when there are errors or changes needed in voter details such as name, age, gender, address, photograph, or EPIC (Voter ID card) number. It can also be used to replace a damaged voter ID card or to mark oneself as a person with disability (PwD) in the electoral database.  

Any voter whose name is already listed in the electoral roll can submit Form 8. It can be either submitted online through the National Voters’ Services Portal (NVSP) or offline by visiting the local Electoral Registration Officer (ERO). Relevant documents along with form 8 must be submitted as proof to support the requested corrections. 

Steps to Fill Form 8 Online

Given below are the steps to fill in your Form 8 online: 

  1. Click on the link https://voters.eci.gov.in/
  2. Under ‘Form 8’ click on ‘Fill Form 8’. 
  3. Enter either your registered email ID, mobile number, or EPIC number followed by your password and the captcha.  
  4. Click on ‘Request OTP’. 
  5. A one-time password (OTP) will be sent to your mobile number. Enter OTP to log in.  
  6. Once you have logged in, you can fill in Form 8 by providing all the relevant details.  
  7. Once you have filled in the details of the changes required in your voter id card, upload the supporting documents and click on ‘Submit’. 
  8. Your documents and application form will be verified. If all the details are correct, then a voter id card with all the updated changes will be issued to you. 

Steps to Fill Form 8 Offline

Given below are the steps to fill in your Form 8 online: 

  1. Click on the link https://voters.eci.gov.in/
  2. Under ‘Form 8’ click on ‘Download’. 
  3. You can download your Form 8 either in English or Hindi. Choose an option suitable for you. 
  1. Once downloaded, take a printout of the application form.  
  2. Fill in the application form making sure all the details mentioned are correct. 
  3. Attach with it all the supporting documents and submit them at the nearest electoral centre. 
  4. Your application and supporting documentation will be examined. You will be issued a voter identification card with all the updated revisions if all the information is accurate. 

Guidelines for Filling Form 8

The guidelines for filling up Form 8 are given below: 

  1. General Instructions: (a) Registered/enrolled electors can apply for a change of residence, correction of entries, replacement of EPIC, or marking as PwD. (b) Upon approval by the Electoral Registration Officer (ERO) for residence change, entry correction, or EPIC replacement without correction, the applicant will receive a new EPIC and must promptly return the old one to the ERO. 
  2. Item No. I (Applicant's Information): The applicant should provide their name, EPIC number, Aadhaar number, mobile number, and email address. Aadhaar number is necessary for authentication. If the applicant doesn't have an Aadhaar number, it should be mentioned in Item I (b). 
  3. Item No. II (Application Option): The applicant must select one of the application options and complete the relevant section. Unnecessary sections should be struck off. 
  4. Application for Residence Change: (a) Address change applications should be submitted to the ERO of the new constituency. (b) The applicant needs to provide their new address, attach a self-attested copy of an address proof document (in their name, parents, or spouse), and tick the relevant document submitted. If none of the listed documents are available, the alternative document name should be mentioned. 
  5. Application for Entry Correction: (a) Applicants seeking correction of existing entries must tick the appropriate box and attach supporting documents, specifying the document name in the space provided. (b) If a change of photograph is desired, a recent, high-quality, unsigned passport-sized color photograph (4.5 cm X 3.5 cm) with a white background must be pasted in the designated box. 
  6. Application for EPIC Replacement Without Correction: The applicant should mark the appropriate box to request a replacement EPIC. They must return their old or damaged EPIC or provide a copy of the FIR/Police report in case of a lost EPIC. 
  7. DECLARATION: The applicant must provide a 'DECLARATION' stating that the information in the application is accurate to the best of their knowledge and belief. It is crucial to note that making any false statements in the DECLARATION is a punishable offense under Section 31 of the Representation of People Act, 1950. Violation may result in imprisonment for a term extending up to one year, a fine, or both.
