What is Form 6?

Form 6 is the authorized application form for including a new voter name in the electoral roll in India. Form 6 is for first-time voters who are 18 years of age, which includes people who have moved to a new constituency, or voters who want to amend or update their details on an existing voter registration. Form 6 can be submitted online through the National Voter Services Portal (NVSP) or offline at the local Electoral Registration Officer (ERO) or Booth Level Officer (BLO) office. Upon submission and validation of information by the ERO, the applicant's name will be recorded in the electoral roll, and a voter ID card may be issued.

The National Voters’ Services Portal (NVSP) was launched by the Election Commission of India in 2015 to provide citizens with a single-window service for voter-related processes.

How to Fill Voter ID Form 6 Online Registration

Voter ID Form 6 is used for new voter registration, updating information, or transferring your voter details to a new constituency. You can complete it online through the National Voters’ Services Portal (NVSP). Follow these steps carefully to ensure your application is processed smoothly: 

Step 1: Access the Portal

Go to the official National Voters’ Services Portal (NVSP). Ensure you are on the official website to avoid fraud. 

Step 2: Register on the Portal

Click on ‘Sign-up’ to create your account. You will need to provide basic details such as your full name, date of birth, mobile number, and email ID. After registration, you will receive a verification OTP to confirm your account. 

Step 3: Log in

Use your registered mobile number and password to log in to the NVSP portal. 

Step 4: Select Form 6

Once logged in, navigate to the ‘Forms’ section and click on ‘Fill Form 6’ to begin your voter registration application. 

Step 5: Complete the Form

Fill in all the required details on Form 6 carefully. The sections include:

  1. State, district, and parliamentary/assembly constituency 
  1. Personal details: Full name, surname, gender, date of birth, and age
  1. Relatives’ details: Name, surname, and relationship with at least one relative (parent, spouse, or guardian) 
  1. Contact details: Mobile number and email ID 
  1. Aadhaar details: Aadhaar number (optional, but recommended for faster verification) 
  1. Address details: Current residential address 
  1. Disability information: Indicate if you have any disabilities and upload a valid certificate if applicable 
  1. Declaration: Date, place, and your signature 

Step 6: Upload Documents

Upload scanned copies of the following: 

  1. Proof of date of birth (any one of the following documents): Birth certificate, Aadhaar card, PAN card, 10th standard certificate, Indian passport, or driving license 
  1. Proof of address (any one of the following documents): Aadhaar card, bank passbook, utility bills, Indian passport, ration card, sale deed, or lease deed 
  1. Passport-size photograph: 4.5 cm x 3.5 cm, front view, with a white background 

Step 7: Preview and Submit

Click ‘Preview and Submit’ to review all the information entered. Ensure all details are accurate. After confirming, click ‘Submit’. An acknowledgement number will be generated. Save this number, as it is needed to track the status of your application. 

Step 8: Download and Offline Submission (Optional)

You can also download a PDF version of Form 6 from the NVSP homepage. Print it, fill it manually, and submit it to your Booth Level Officer (BLO) if you prefer offline registration. 

How to Check Form 6 Application Status 

  1. Step 1: Log in to the NVSP portal. 
  1. Step 2: Click on ‘Track Application Status’.
  1. Step 3: Enter your application reference number and select your state. 
  1. Step 4: Click ‘Submit’ to view the status of your application.

Important Notes

  1. Form 6 can be submitted anytime during the year. 
  1. You can submit the form online or offline at your local Assistant Electoral Registration Officer or Electoral Registration Officer office.
