The EPF Form 20 is used by the nominee, legal heir, or guardian of a deceased member of the EPF to withdraw the Provident Fund balance. Please follow these steps to fill in and submit the form correctly.
Step 1: Download the Form
Go to the EPFO website and download EPF Form 20. It is also available in printed form from your last employer or the regional EPFO office.
Step 2: Collect Documents
You will need the death certificate, the cancelled cheque of the claimant’s bank account details, and other relevant related forms including Form 5IF, Form 10D and Form 10C depending upon the claim that you plan to submit.
Step 3: Fill Member details
Provide the full name of the deceased member, name of the deceased individual's father or husband, details of the last employer, EPF account number, the date the member left service, the reason for leaving (enter ‘Death’), the date of death, and marital status (widowed or unmarried) for the deceased at time of their death.
Step 4: Fill Claimant Details
In this section, provide details of the claimant's full name, gender, age, marital status, and the relationship of the claimant to the deceased member. If the claim is to be completed as a guardian, the guardian's name and details should be included along with the full postal address in block letters.
Step 5: Select Mode of Payment
From the above options, please select electronic fund transfer (ECS) as the preferred method of repayment, by attaching the cancelled cheque for quicker and safer payment.
Step 6: Sign and Get Attested
Both the claimant and the employer must sign every page of the form. If the employer is unavailable, the form must be attested by an authorised official such as a magistrate, gazetted officer, bank manager, or village panchayat president.

EPF Form 20 can be filled in by a family member, legal heir, or nominee after a member's death. If the nominee is a child, or physically or mentally unwell, a guardian can apply on behalf of the child or the physically or mentally unsound person.
