In case you have forgotten your username and password, there is an option for changing it. If you cannot remember your user ID, you will be required to click on 'Forgot Login Name?' and then enter your policy number and birth date. Once you click on the 'Submit' button, you will be redirected to a page wherein you will be able to create a new user ID.
If it is the password that you have forgotten, simply click on the 'Forgot Password?' option and enter your login name/user ID. You will thereafter be required to follow a few simple steps for changing your password.
