Bonus
  • Every claim free year will result in 5% increase in the hospitalization cover limit up to 50%
  • Claim free bonus is not applicable for non-hospitalization cover
Claim procedure
  • Cashless hospitalization at 2600 network hospitals
  • Claims are accepted only after three months of policy commencement
  • Pre-authorisation is mandatory for planned hospitalization. Customers have to fax to 1800 425 2200 or call 1800 22 5544 (toll free)
  • Customers have to make payment at non-network hospitals which will be subsequently reimbursed upon submission of required documents
  • For emergency hospitalization, Chola MS has to contacted within 24 hours
  • Bills with diagnostic reports and prescriptions have to be submitted
Exclusions
  • Pre-existing conditions till four years of coverage from health insurance policy commencement
  • Medical expenses in the first months of policy commencement except accidents cases.
  • First year of insurance: Hernia, Benign Prostate Hypertrophy, Tonsillitis, Fistula in Anus, Piles, Congenital diseases
  • First two years of insurance: Hysterectomy (non-malignant) gastric ulcer, knee replacement surgery, tumours, spondylosis chronic kidney diseases, varicose veins, goitre, fibroid in uterus, rheumatism, prolapsed uterus deviated nasal septum, sinusitis, cataract, arthritis, glaucoma and mastoidectomy
